Nine fields, one twenty minute call, then a yes or a no with a start date.

No sequence, no demo, no discovery process with three stakeholders. If it is a fit you get the tier, the number, and the date we can start. If it is not, you hear that on the first call instead of in month two.

What you need

This form is a layout preview and is not connected yet. Nothing is transmitted or stored. On the live site it will route to the carrier intake queue with a same business day acknowledgment.

What happens next
1

Acknowledgment

Same business day, from a person, with the document list attached.

2

Qualification call

About twenty minutes. Equipment, insurance, lanes, and what you need the desk to do.

3

A straight answer

Yes with a tier and a start date, or no with the reason. Nobody gets left on a maybe.

Before the call, have these ready

  • ·Certificate of insurance with limits and effective dates
  • ·Equipment list with VINs and last annual inspection dates
  • ·Driver CDL and medical certificate expiry dates
  • ·Whatever you know about UCR, IFTA, IRP, and Form 2290 status

If some of that is missing or overdue, say so on the form. Overdue filings are the reason the compliance intake exists, and they are not a disqualification unless the truck should not be loaded today.

Privacy, plainly. What you send is used to evaluate the fit and prepare an agreement. Carrier data is never sold, never shared with brokers or lead buyers, and never added to a marketing list unless you ask for it.

Skip the form

If you would rather just call, call. It is the same twenty minute conversation.

The form exists so you do not have to explain the operation twice. It is not a gate. If you are sitting at a truck stop with time to talk, the phone is faster, and the person who answers is the person who would run your desk.

Reasonable questions to open with
  • ·What would you have done differently with my last three loads?
  • ·Which of my filings is closest to being late right now?
  • ·What is my fee at the gross I actually ran last month?
  • ·Who answers when my driver calls at four in the morning?
  • ·What happens to my files if I leave?

The last one has a short answer. Your records are yours, exported in full, and the M365 tenant stays on your own domain.
